Forms to be attached with the enrolment form:

Child at school

  • Enrolment form

  • Child's last report from previous pre-school or primary school

  • Certified proof of address 

  • Certified copy of mother's ID

  • Certified copy of father's ID 

  • Certified copy of ID of person responsible for the school account (if not a parent)

  • Certified copy of child's birth certificate

  • Vaccination card of the child

  • Financial clearance certificate

  • Proof of registration fee payment 

  • Completed debit order form (payment monthly in advance before the first day of the month to qualify for the early payment discount)
